Seniors Need More Than Insurance

India is at a demographic turning point. By 2050, it is expected to have over 319 million people aged 60 years and above, making it one of the largest ageing populations in the world. The question is no longer whether India will become an ageing society, but whether it will become a society that ages well.

When we think about ageing, the conversation usually begins with health issues, expenses, and health insurance. It’s certainly an important issue, as healthcare costs continue to rise and out-of-pocket expense can be high. But ask any family caring for an ageing parent, and their concerns go beyond insurance. Who will accompany them to the doctor? Is there a trained caregiver after surgery? Can they safely navigate their neighbourhood? Who will they talk to if they are alone?

Insurance protects against financial shocks. But it cannot ensure healthy, independent and dignified ageing. As India ages, we must move beyond financing illness towards creating systems that enable people to stay healthy, independent, and engaged in their later life.

The World Health Organization (WHO) defines healthy ageing as developing and maintaining the functional ability that enables wellbeing in older age. The UN Decade of Healthy Ageing (2021-2030) builds on this vision through four global priorities—combating ageism, creating age-friendly environments, delivering integrated person-centred care, and expanding long-term care. What is central to this vision is helping older adults continue living safely, independently, and with dignity in their own homes and communities for as long as possible.

A paper on active and healthy ageing by the FICCI Task Force says that India should view ageing not as a welfare challenge, but as an opportunity to unlock a “longevity dividend”, where healthier and active seniors continue contributing economically and socially, while reducing long-term healthcare costs.

Why Prevention Is Best

Our healthcare system remains largely organised around treating illnesses rather than preventing it. Yet many age-related conditions—including frailty, falls, hypertension, diabetes complications, and even cognitive decline—can often be delayed through preventive healthcare, regular activity, healthy nutrition, and timely screenings.

Wellness benefits offered by insurers are welcome, but healthy ageing needs neighbourhoods with accessible parks, walking spaces, and stronger primary healthcare facilities.

The FICCI knowledge paper estimates that investments in preventive healthcare alone could generate enormous savings by reducing avoidable hospitalisation.

Where You Live

Most older Indians prefer to live in their own homes in old age. But our homes, cities and neighbourhoods are rarely designed for aged living.

Uneven footpaths, inaccessible public transport, unsafe road crossings, and buildings without lifts increase the risk of falls and disability, and overall exclusion.

Age-friendly housing should become a part of mainstream urban planning through barrier-free design, ramps, elevators, handrails and accessible public spaces. Equally important is expanding community-based services, home healthcare, rehabilitation, dementia support, and assisted living options that allow seniors to remain connected to their families and communities.

Healthy ageing is not only a health sector issue, but also an urban planning, housing, transport and local governance issue.

Looking Beyond Insurance

For most middle-class families, the financial burden of ageing extends well beyond hospitalisation. Home nursing, medicines, diagnostics, physiotherapy, rehabilitation, assistive devices and long-term care often become recurring expenses.

Health insurance, though necessary, is only one component of financial protection. Equally important are affordable outpatient services, home-based care, rehabilitation, palliative care, assistive technologies and stronger primary healthcare that reduce the need for expensive hospitalisation.

Behind every ageing individual is often an unpaid family caregiver, mostly a woman. Supporting caregivers through training, respite services, special leave and flexible workplace policies is not only socially desirable, but economically essential, as they form the backbone of India’s long-term care system.

Technology can also become a powerful enabler. Telemedicine, wearable health devices, digital health records and artificial intelligence (AI)-enabled home monitoring can help older adults remain independent for longer. But these innovations will succeed only if accompanied by greater digital literacy and age-friendly technology.

Purpose And Social Connection

The most overlooked determinants of healthy ageing are purpose and social connections. The Harvard Study of Adult Development, one of the world’s longest-running longitudinal studies, shows that strong relationships and meaningful social connections are among the strongest predictors of health, happiness and longevity. Conversely, loneliness and social isolation are associated with increased risks of depression, cardiovascular disease, cognitive decline and premature mortality.

Studies have shown that adults who volunteer for 100 hours a year, in community work or as tutors and mentors in schools, enhance their physical health, cognitive functioning and emotional wellbeing, besides improving educational outcomes for children. India possesses an enormous but underutilised asset in its millions of retired professionals. Structured opportunities for volunteering, mentoring, tutoring, entrepreneurship and others can transform older adults from passive recipients of care into active contributors to society. The essence of the “Longevity Dividend” is recognising older people as contributors rather than dependents.

Four Pillars For India's Aged

Preparing India for an ageing society requires action across government, industry, and civil society.

First, invest in healthy ageing. Shift from treating illness to promoting lifelong wellness through prevention, primary care, nutrition, mental health and regular health screening.

Second, build age-friendly homes, neighbourhoods, cities, transport systems and digital services that allow older adults to live independently, safely and with dignity in their own communities.

Third, unlock the silver economy and the longevity dividend. Create opportunities for flexible work, entrepreneurship, mentoring, volunteering for seniors.

Fourth, expand affordable health insurance alongside home healthcare, rehabilitation, and long-term care through coordinated action across ministries, local governments, the private sector and civil society.

India’s ageing population should not be viewed as a future burden but as one of its greatest untapped assets.

Healthy ageing depends on communities that support independence, policies that promote dignity, and opportunities that enable older people to remain healthy, connected and purposeful.

If India embraces this broader vision of active and healthy ageing, it will unlock one of the country’s greatest opportunities: the Longevity Dividend.
