GST Relief On Health Insurance: Room-Rent Caps Can Still Cut Your Claim

Individual health policies no longer carry 18 per cent Goods and Services Tax (GST), removing the tax burden on premiums. But the exemption does not remove room-rent caps or the deductions that can follow when a patient chooses a costlier room.

If a policy allows a Rs 5,000 room but the patient chooses one costing Rs 10,000, the eligible room rent is 50 per cent of the actual amount. Where a proportionate-deduction clause applies, some related medical expenses may also be paid in that ratio.

The patient’s expense can therefore extend beyond the difference in room rent. How much more depends on the policy wording and whether the hospital charges differently across room categories.

How A Costlier Room Affects The Claim

“Depending on the policy wording, this can result in proportionate deductions on associated expenses such as nursing, doctor or specialist fees, and operation-theatre charges. The exact expenses covered by the clause should always be checked in the policy,” says Minoo Mantri, executive vice president - insurance, Anand Rathi Share and Stock Brokers.

Take a five-day stay in a Rs 10,000 room against a Rs 5,000 daily limit. The policyholder pays the Rs 25,000 difference in rent. If the eligible surgeon and operation-theatre charges total Rs 1.2 lakh and attract the same deduction, the insurer would pay Rs 60,000 towards those charges.

“The actual deduction will depend on the policy terms and on whether those charges increase because the patient has chosen a higher room category,” says Abhishek Bansal, CEO, insurance business, InsuranceDekho.

This does not mean that 50 per cent will be cut from the entire hospital bill. Pharmacy bills, consumables, implants, medical devices and diagnostic tests are generally kept outside such deductions. ICU charges are treated separately. Being outside proportionate deductions, however, does not establish whether an expense is otherwise covered by the policy.

“If a hospital bills surgeon fees or operation theatre (OT) charges at a flat rate across all rooms, the insurer shouldn't deduct on those items, and the policyholder has every reason to question it if it does,” says Shivendra Pancholi, executive director, Coverfox, an insurance distribution technology platform.

An itemised bill helps identify which charges vary with room category. A bundled package rate makes it harder to separate what can be deducted from what cannot.

Check Room Eligibility Before Admission

Start with the policy wording. Check whether room rent is capped at a fixed amount, a percentage of the sum insured, or a room type such as a single private air-conditioned (AC) room.

“Customers should look beyond the sum insured and check the room-rent and ICU limits, proportionate-deduction clause, sub-limits, co-payments and the definition of associated medical expenses,” says Mantri.

Disease-wise sub-limits and other caps also need attention. Before a planned admission, obtain an estimated bill and confirm room eligibility with the insurer or third-party administrator during pre-authorisation.

“Before a planned admission, it is useful to ask the hospital whether doctor fees, nursing charges or procedure costs change with the room category,” says Bansal.

“If no room in your eligible category is free, get that noted in the file. Some insurers treat a forced upgrade differently from a chosen one,” says Pancholi.

Ask for confirmation of the eligible room in writing. Knowing the permitted category and the hospital’s tariffs before admission can help explain how much the patient may have to pay at discharge.
